Scenic 1.9 dci power loss after gearchange

Hi,
My 2002 Scenic 1.9 dci hesitates after each gearchange, this is much more pronounced when the engine is cold and very bad during or after heavy rain.
Another problem that may be linked, at altitude, say above 1500 metres, the car is very slow to pick up revs even if I floor the accelerator.

The rest of the time the car runs fine and returns 49mpg overall. It has done 185k miles.
