Kangoo gearbox problems

Hi there I have 2005 1.5dci kangoo van . It is getting very "notchie"going into most gears except reverse and 5th. Could anybody tell why this is happening . I've changed the gear oil twice but it's still the same. And could anybody tell where I can find the gearbox code for my van as I can't find it anywhere. I have a scrap van so if the worst comes to the worst I will swap the box . But I need the code first.
